Predictable Care: To make your annual health review smoother and easier to remember, we use a "birth month" recall system. If you have a long-term condition (like diabetes, asthma, or high blood pressure), your review will be scheduled during the month of your birthday.
Step 1: The Invitation: You will receive an invitation (usually via text message) during your birth month asking you to book your initial checks.
Step 2: The Pre-Check: Before seeing a doctor or nurse, you must get essential checks done (e.g., blood tests, urine tests, or blood pressure).
Step 3: The Review Appointment: Once results are back, you attend the main review to discuss your medications and agree on a plan for the year ahead.
Important Note
While this system organises your routine care, you should never wait for your birth month if you feel unwell. If you are worried about your condition at any other time of the year, please contact us immediately.
