Welcome To Stonefield Surgery

Key Information

  • Welcome: Whether you are new to Rochdale or joining our practice for the first time, welcome. We are really glad you are here.

  • Our Care: Our team provides care for the whole family, from everyday health concerns and childhood immunisations to managing long-term conditions and specialist clinics.

Step 1: The Health Hub

If you have a new symptom or health question, your first step should be to check the Health Hub on our website.

The Health Hub contains trusted advice from our clinicians about common symptoms, conditions, and treatments. It includes short videos and guides explaining:

  • When you can safely manage a problem yourself.

  • When to contact the practice.

  • When to seek urgent help.

Step 2: Contacting Us Online (Patchs)

If you do need to contact us, the easiest way is through Patchs, our online request system.

  • How to Access: You can access it directly through our website or the NHS App.

  • The Benefit: It is often much quicker than waiting in a phone queue.

  • (Note: Our phone lines remain available for urgent problems or for patients who cannot use online services).

Step 3: The NHS App

The NHS App is the absolute simplest way to manage your healthcare from your smartphone or tablet. You can use it to:

  • Order repeat prescriptions.

  • View your test results.

  • Check your appointments.

If you don’t have the NHS App, we have created a page below with more information, including how to download it.

Managing Long-Term Conditions

We run regular reviews for patients with ongoing health conditions.

If you have a long-term condition (like asthma, diabetes, or high blood pressure), we will usually invite you for a yearly review during your birth month so we can keep your care and treatment plan up to date.

We Are Here to Help

Whatever your health needs, our team is here to support you.
